People with Parkinson disease (PD); Mild/Early, Moderate/Middle, Severe/Late stagesTest description:
- Uses combination of patient self-report and physical examination
- Contains questions related to underlying impairments (11 items), activities of daily living (ADLs) (10 items), and cognition/affect (3 items)
- Each item scored on a 0-4 ordinal scale, with higher scores representing greater severity (0=no problem, 4=an important issue to the patient)
